Append an event to a loan's history — a rate lock, an appraisal update, a
sync marker.

Stage changes are not posted here. Use
PATCH /api/partner/loans/{id}/stage, so that the loan's stage and its
history stay in step. Posting stage_changed to this endpoint is rejected
with a message saying so.
